For Father’s Day this year I wanted to create something sentimental that my kids could create themselves. I have always loved reading the hilarious answers that our daughter gives when she has done questionnaires at school. It is so funny to hear children’s opinions and observations about their parents – they are usually brutally honest and hilarious at the same time π
I created this Father’s Day Questionnaire the other day, and I wanted to share it with all of you! You only need a couple materials to create this memorable keepsake. It is super easy, quick, and would make a wonderful addition to a Father’s Day gift!

Materials needed:
- Cardstock
(I use this paper for my printables and I love it!)
- Home Printer
- Pencil/Pen/Colored Pencil/Crayon
All you need to do is download your free printable here, print it out on your home printer, and give your child their writing utensil of choice. My 6 year old prefers to use colored pencils at the moment, but any kind of writing utensil will do.
You can place this keepsake in a picture frame, or even use a 3-hole punch and add it to a binder for safe keeping. If you wanted to make this a yearly tradition, it would be so fun to keep them in a binder together and watch as their writing and answers change over the years!
NOTE: This printable is 8.5 inches x 11 inches. If you will be adding this keepsake to a frame, you can place it in an 8.5×11 document frame. Or you could also trim it to 8×10 inches to fit a standard 8×10 frame.
